- Attested copy of Secondary examination mark sheet or certificate.
- Self-signed passport size latest photo on the application form (do not staple).
- Attested copy of caste certificate.
- Self-certified copy of Aadhaar card/Ration card/Voter ID card/Original residence or any other equivalent certificate for verification of being a local resident of the village falling under the jurisdiction of the Panchayat Samiti for which the application is being made.
- Self-certified affidavit that no lawsuit is pending.
- Experience certificate (if any).
