- Internship completion certification (For GDMOs)
- PG Diploma/PG Degree Mark Sheets and Passing Certificate
- Mch/DNB (Super Specialty) / DrNB Certificate (for Super Specialist)
- PG Registration Certificate (MCI or NMC or SMC)
- Experience Certificate showing relevant post qualification experience
- NOC from present employer, if employed under PSUs/Autonomous Bodies/Govt. Department
- Photo Identity Proof (Aadhaar Card, PAN Card, Driving License, Passport, Voter ID)
- Address proof (Voter ID, Driving License, Aadhaar Card or others)
